The MDS/Care Plan Coordinator is responsible for the timely, accurate completion of the federally required Minimum Data Set Assessment and the Care Plan. The MDS Coordinator decides which programs will be included on the Care Plan and summarizes the rational for this decision. The MDS Coordinator conducts the bi-monthly Care Plan Conferences. The MDS Coordinator reports to the Extended Care Facility Director of Nursing and/or the Nursing Home Administrator.
Minimum LPN
Preferred MDS certification or willingness to obtain
Preferred at least one year experience with MDS-Care Plan
1-3 years of geriatric experience
Current Illinois Licensure
BLS certified through AHA required.

The pay grade for this position is 3200 for LPN and 4100 for RN. Compensation InformationAll wages are determined by experience and skills; wage information will be discussed further upon interview selection.
