The Hospice Home Care Nurse reports to the Team Manager. He/she is responsible for identifying patient/family/facility care needs, and for providing nursing and supportive care for the terminally ill patient and their families. Education required includes Diploma, ADN and/or BSN from an accredited institution. Licensure requires a current Maryland State license as a Registered Nurse. Experience required is two years of nursing experience or equivalent combination of education and experience. Skills needed include knowledge of universal precautions for infection control, strong time management, excellent interpersonal and communication skills, patient and family focus, and skill in the use of EMR or ability to learn. Principal duties include completing initial assessments, preparing and implementing care plans, providing direct nursing care, supervising HHA care delivery, cooperating with Clinical Manager and interdisciplinary team, problem-oriented charting, teaching family and caregivers patient care skills, ensuring equipment availability, attending to patient and family at time of death, documenting in facility chart, maintaining relationships with facility staff, reconciling medications, updating staff and family on visit schedules, attending care plan meetings, providing educational in-services, and participating in marketing and customer service activities. The role requires COVID-19 vaccination or approved exemption prior to employment.
